Our Privacy Policy

This privacy policy is to provide information to you, our patient, on how your personal information (which includes your health information) is collected and used within our practice, and the circumstances in which we may share it with third parties.


What we collect?

The information we will collect about you includes your:

  • names, date of birth, addresses, contact details.
  • medical information including medical history, medications, allergies, adverse events, immunizations, social history, family history and risk factors. 
  • Medicare number (where available) for identification and claiming purposes. 
  • healthcare identifiers & health fund details.


How we collect your information?

Our practice may collect your personal information in several different ways.

When you make your first appointment our practice staff will collect your personal and demographic information via your registration. Please let us know if you do not wish to share this information.

During the course of providing medical services, we may collect further personal information. 

[Information can also be collected through electronic transfer of prescriptions (eTP), My Health Record, eg via Shared Health Summary, Event Summary.

We may also collect your personal information when you visit our website, send us an email or SMS, telephone us, make an online appointment or communicate with us using social media. 

In some circumstances personal information may also be collected from other sources. Often this is because it is not practical or reasonable to collect it from you directly. This may include information from:

  • your guardian or responsible person
  • other involved healthcare providers, such as specialists, allied health professionals, hospitals, community health services and pathology and diagnostic imaging services
  • your health fund, Medicare, or the Department of Veterans’ Affairs (as necessary). 


Dealing with Us anonymously.

You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are required or authorised by law to only deal with identified individuals. 


User Rights

You have the right to request access to, and correction of, your personal information.

Our practice acknowledges patients may request access to their medical records.

We require you to put this request in writing. Contact us via phone or email if you need to make a request. 

Our practice will respond within a reasonable time to process your request. Note: it can take up to 30 days to collate the information that you may require.

Our practice will take reasonable steps to correct your personal information where the information is not accurate or up to date.

From time to time, we will ask you to verify that your personal information held by our practice is correct and current.

Retention

We will retain your personal information for as long as necessary to provide our services, and as necessary to comply with our legal obligations, resolve disputes, and enforce our policies. Retention periods will be determined taking into account the type of information that is collected and the purpose for which it is collected, bearing in mind the requirements applicable to the situation and the need to destroy outdated, unused information at the earliest reasonable time. Under applicable regulations, we will keep records containing client personal data, account opening documents, communications and anything else as required by applicable laws and regulations. 

We may rectify, replenish or remove incomplete or inaccurate information, at any time and at our own discretion.

Once you request to transfer to another clinic for ongoing care and no longer wish to be seen by our doctors, your information will be made ïnactive”in our system.


How we safeguard your information’?

Your personal information may be stored at our practice in various forms e.g. as paper records, electronic records, visual records (X-rays, CT scans, videos and photos), audio recordings.] 
We store your information electronically, in protected Information systems.
 Any hard copies are scanned into electronic files and the hard copies are destroyed.
The access of electronic information is restricted and only made accessible to the health professionals or admin staff who may need to access your records for administrative purposes.


When and why, we may share your information?

We sometimes share your personal information:

with third parties who work with our practice for business purposes, such as accreditation agencies or information technology providers – these third parties are required to comply with APPs and this policy. 

  • with other healthcare providers
  • when it is required or authorised by law (e.g. court subpoenas) 
  • when it is necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent
  • to assist in locating a missing person
  • to establish, exercise or defend an equitable claim
  • for the purpose of confidential dispute resolution process 
  • when there is a statutory requirement to share certain personal information (e.g. some diseases require mandatory notification)
  • during the course of providing medical services, through eTP, My Health Record (e.g. via Shared Health Summary, Event Summary).

Only people who need to access your information will be able to do so.
Other than in the course of providing medical services or as otherwise described in this policy, our practice will not share personal information with any third party without your consent.

We will not share your personal information with anyone outside Australia (unless under exceptional circumstances that are permitted by law) without your consent.

Our practice will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent, you may opt out of direct marketing at any time by notifying our practice in writing. 

Our practice may use your personal information to improve the quality of the services we offer to our patients through research and analysis of our patient data. 

We may provide de-identified data to other organisations to improve population health outcomes. The information is secure, patients cannot be identified and the information is stored within Australia. You can let our reception staff know if you do not want your information included. 

Please note that even if you have unsubscribed from receiving marketing emails from us, we may send you other types of important e-mail communications without offering you the opportunity to opt out of receiving them. These may include any urgent health related news that may have an impact on the community.

Minors

We understand the importance of protecting children’s privacy, especially in an online environment. The Site is not designed for or directed at children. Under no circumstances shall we allow use of our services by minors without prior consent or authorization by a parent or legal guardian. We do not knowingly collect Personal Information from minors. If a parent or guardian becomes aware that his or her child has provided us with Personal Information without their consent, he or she should contact us using the details below.


Use of images

Images used on the website (clinical, non-clinical and photos)

All client images are only used after consent is obtained from the client.

No Images- client photos, clinic images, treatments photo and picture of staff can be reproduced in any manner* and remain property of Plaza Central Medical and Aesthetics.

* In any other websites, for any other marketing or clinical images by anyone including journalism or reporting (without prior consent). apart from Plaza Central Medical and Aesthetics.

Photos taken during surgical procedures are stored in the patients file as part of record keeping.


How can you lodge a privacy-related complaint, and how will the complaint be handled at our practice?

We take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing. We will then attempt to resolve it in accordance with our resolution procedure.

Resolution

Upon the receipt of a concern, our privacy officer will review the nature of your concern and respond to you via your preferred communication methods.

All response will be done within 24 hours of receiving your request, on a business day.


Updates and amendments to the policy

We reserve the right to periodically amend or revise the Privacy Policy; material changes will be effective immediately upon the display of the revised Privacy policy. The last revision will be reflected in the "Last modified" section. Your continued use of the Platform, following the notification of such amendments on our website, constitutes your acknowledgment and consent of such amendments to the Privacy Policy and your agreement to be bound by the terms of such amendments.

We also use a tool called “Google Analytics” to collect information about your use of the Site. Google Analytics collects information such as how often users access the Site, what pages they visit when they do so, etc. We use the information we get from Google Analytics only to improve our Site and services. Google Analytics collects the IP address assigned to you on the date you visit sites, rather than your name or other identifying information. We do not combine the information collected through the use of Google Analytics with personally identifiable information. Google’s ability to use and share information collected by Google Analytics about your visits to this Site is restricted by the Google Analytics Terms of Use and the Google Privacy Policy .
